"Zero AIDS" Initiative: How Did a Community-Wide Project Help Reduce New HIV Diagnoses by 40% in the Alpes-Maritimes Region Over Four Years?

Introduction - The Alpes-Maritimes department is one of the departments most affected by the HIV epidemic in France. It saw a 54% increase in diagnoses between 2010 and 2014. This article describes the initiatives led by Corevih Paca-Est and local stakeholders, including the creation of the unifying project "Objectif Sida Zéro" (OSZ), to accelerate the fight against this alarming epidemiological situation. Materials and Methods - Based on a project-based methodology with a key population approach, OSZ’s initiatives have focused since 2016 on the rollout of pre-exposure prophylaxis (PrEP), improving access to care, and reducing the time to treatment initiation. Primary care professionals were trained in comprehensive prevention (use of condoms, screening, and preventive and curative treatments), and the collection of epidemiological indicators was optimized. Results - Since 2015, the number of new annual diagnoses has fallen by 40%, reaching a rate of 107 cases per million inhabitants in 2018 (95% CI: [93–122]). The decline is particularly pronounced among people born in France, especially among men who have sex with men (MSM) (-61%), but does not appear to apply to people born abroad, particularly women. Discussion-Conclusion - The initiation of antiretroviral treatment upon diagnosis, recommended in late 2013, the promotion of diversified prevention starting in 2015, and actions by civil society organizations have contributed to a sharp decrease in new HIV diagnoses, particularly among MSM born in France. Efforts must, however, be intensified regarding people born abroad who are exposed to HIV. The continuation of coordinated actions at the local level, the inclusion of new partners (clinical laboratories, pharmacies, regional healthcare professional communities, etc.), and regular estimates of the undiagnosed epidemic are all necessary levers for achieving the goal of zero new infections by 2030.
