Influenza outbreaks in two nursing homes in the Bouches-du-Rhône department, France, March–April 2008.

In April 2008, two influenza outbreaks occurred in two nursing homes (Ehpad) that shared staff. An investigation was conducted among all residents and staff at both nursing homes. Samples were collected to confirm suspected cases of influenza and to investigate other possible causes. The attack rates among residents and staff were 42% and 10% in Ehpad 1, and 28% and 11% in Ehpad 2. Of all cases, 6 were confirmed by rapid diagnostic test (RDT) for influenza; 15 residents were hospitalized, and 3 died. Influenza vaccination coverage was 81% among residents and 17% among staff. Lack of knowledge regarding proper procedures and the absence of a coordinating physician led to delayed diagnosis and unnecessary hospitalizations in Nursing Home 1. In Nursing Home 2, a more responsive approach helped contain the outbreak. Although this could not be biologically demonstrated, staff members common to both facilities most certainly contributed to the spread of the outbreak. Staff vaccination should be promoted, and during outbreaks of acute respiratory infections in nursing homes, sick staff should be sent home or, failing that, required to wear face masks to reduce the spread of influenza within the facility.
