Evaluation of a nutritional guidance and counseling program provided by occupational physicians to overweight or obese employees in Upper Normandy

The Haute-Normandie region implemented a nutritional counseling and guidance initiative as part of the National Nutrition and Health Program. The initiative was carried out by occupational physicians between April 2004 and January 2007. The main objective of the study was to assess the impact of this initiative on the body mass index and behaviors of overweight or obese employees. The occupational physician provided a specific informational letter on overweight and obesity to overweight and obese employees and offered them a referral letter for treatment of overweight or obesity. All employees received the guide "Health Starts with What You Eat." Participating employees were interviewed during two consecutive annual consultations. The control group, consisting of employees receiving standard occupational health consultations, participated in both data collection rounds. The study showed that the main factors linked to overweight and obesity were age and gender: 32% of women were overweight or obese, compared to 47% of men. Furthermore, a relationship between overweight and obesity and company size was identified (employees working in companies with 100 to 299 employees were more often overweight or obese), as well as with the type of employer-provided lunch arrangement. The study did not show a relationship between overweight and obesity and work schedules or the composition of the lunch meal. Employee response to this initiative was positive, as only 8% of participating employees declined the specific informational letter. This study demonstrated the feasibility of occupational physicians intervening with employees on a public health topic. However, the one-time nutritional counseling and guidance initiative evaluated did not demonstrate its effectiveness. The study identified groups of employees for whom a targeted intervention—likely as part of a more comprehensive care plan—could be considered. (R.A.)
