Tuberculosis in the Grand Est region. 2023 Report.

Key Points

Online Reporting

  • In the Grand Est region, the proportion of tuberculosis cases reported electronically (via the e-DO app) remains below the national rate.

Trends in Tuberculosis Incidence

  • The reporting rate, which had been declining since 2019, stabilized in 2023 at 4.4 cases per 100,000 inhabitants.

  • At the subregional level, the highest reporting rates are observed in the departments of Haute-Marne (though with a low number of cases) and Haut-Rhin.

  • The incidence of tuberculosis remains twice as high among men.

  • Since 2019, it has been declining among adults aged 25 to 64, who account for half of all cases.

Main clinical and epidemiological characteristics of tuberculosis

  • Cases of pulmonary tuberculosis, whether isolated or associated with extrapulmonary sites, are the majority (71%) and are bacilliferous—and therefore more contagious—in half of cases.

  • The incidence of severe forms (meningoencephalitis or miliary tuberculosis) remains low (5% of cases) and has been stable in recent years.

  • Reported cases of active tuberculosis primarily involve highly vulnerable individuals who have recently arrived from areas or countries with high tuberculosis endemicity.

Multidrug resistance

  • The incidence of tuberculosis cases resistant to the two main first-line antituberculosis drugs (isoniazid and rifampicin) remains low (fewer than 5 cases per year) and stable in the Grand Est region.

Treatment Outcomes

  • In terms of tuberculosis control performance indicators, information on treatment outcomes remains insufficient (only one in two cases reported).

  • Treatment completion rates have exceeded 80% since 2019 at the regional level, and in 2023 will still meet the WHO’s treatment success target (85%).
