Santé publique France is conducting an epidemiological study on the health and quality of life of residents living near the Lacq industrial area, as a follow-up to the work already underway in this area since 2016. The survey questionnaire is presented in its entirety, with the exception of control questions, which have been omitted to make it easier to read. It includes the instructions displayed on the interviewers’ screens as well as the conditions for each question
