Impact of Changes in BCG Vaccination Practices on the Epidemiology of Tuberculosis in France in 2009
The mandatory BCG vaccination requirement was suspended in July 2007 and replaced by a recommendation to vaccinate children at risk of tuberculosis. Furthermore, the intradermal vaccination device (Monovax®) was withdrawn from the market in January 2006, leading to a decline in vaccination coverage from that date onward. Data from the mandatory reporting of tuberculosis among children born after January 2006 allow us to assess the impact of these new vaccination protocols. In 2009 in France, the number of tuberculosis cases among children under 4 years of age was similar to that observed before 2006, and the number of severe forms (meningitis and miliary tuberculosis) remained very low. However, the number of cases among children under 4 is increasing outside the Île-de-France region and is accompanied by an increase in the proportion of unvaccinated cases. Epidemiological data do not currently show any impact of the new vaccination protocols beyond what was expected, but changes in the epidemiology of tuberculosis among children and vaccination coverage levels among at-risk children must be monitored with the utmost attention.
