The effectiveness of smoking cessation clinics in France between 2011 and 2013.

Objectives: France is one of four European countries to offer a comprehensive smoking cessation program that includes a helpline, nicotine replacement therapy (NRT) with a reimbursement package, and tobacco dependence counseling. This study evaluates the effectiveness of counseling sessions under the CDTnet program. Methods: Data from 23,810 adult patients followed between 2011 and 2013 were entered into the CDTnet database. Our primary outcome measure was abstinence maintained for one month, validated by exhaled CO measurements, among the 10,116 patients who quit smoking during follow-up. Predictors of abstinence were determined using a multivariate regression model. Results: The rate of abstinence maintained for one month was 45.2%. The abstinence rate reached 47% with a prescription for a combination of NRT and 53% with a prescription for varenicline. The beneficial effect of treatment increased significantly with the number of consultations. Unemployed individuals were just as likely to be abstinent as employed individuals (OR=0.88; 95% CI: [0.75–1.04]). Similarly, the level of tobacco dependence had no impact on abstinence. However, being aged 18–24, having a history of alcohol misuse, or daily cannabis use were associated with lower abstinence rates. Conclusion: With appropriate care, even the most dependent individuals or certain groups of people in precarious situations can successfully maintain abstinence. This assessment offers very encouraging prospects for individualized smoking cessation support, while highlighting the need for specific care approaches for certain smoking populations.
