“One Health” Surveillance and Prevention of Antibiotic Resistance in France: 2016–2022 Report from the Annual Synthesis Coordinated by Santé publique France

The "One Health" antibiotic resistance report, coordinated by Santé publique France and published annually during Antibiotic Awareness Week, is part of an initiative launched in the late 1990s to combat antibiotic resistance and promote the proper use of antibiotics. After two editions dedicated specifically to human health, the document incorporated animal health and the environment in 2016, as part of the 2016 interministerial roadmap. The indicators selected for the summary based on their availability and relevance to the human and animal sectors focus on antibiotic consumption and resistance to third-generation cephalosporins and fluoroquinolones in Escherichia coli. For the environment, the presence of antibiotics or their metabolites, as well as the presence of bacteria and resistance genes in various environmental compartments, focuses in particular on marker genes indicating the presence of E. coli in the environment and its resistance profile. Since 2016, antibiotic consumption has been declining. This decrease has been very gradual (-7.8% between 2016 and 2019) in human health and more significant in animals (-19% between 2016 and 2019), following a drastic decrease in animal use between 2011 and 2016 (-37%). During the same period, initiatives promoting the appropriate use of antibiotics have multiplied, driven by a network of stakeholders that is rapidly taking shape. Despite this encouraging trend, environmental contamination by antibiotic residues is common and often reaches significant levels. This public-facing document is intended for non-expert healthcare professionals to inform them of the latest developments and trends in the complex field of antibiotic resistance prevention. Its development has helped bring together stakeholders who are now developing integrated analyses. Raising public awareness and improving the availability of environmental data are the next challenges to be addressed.
