Teaching patients with diabetes how to manage their condition.
The Center for Diabetes and Nutritional Diseases Education, which opened in Roubaix in the 1980s, focuses its work—which draws on the multidisciplinary expertise of its team—on combining medical care with education to help people become self-reliant in managing their condition. An assessment of the program’s results, compared to those of a traditional healthcare facility, reveals a positive impact in terms of reduced numbers of hospitalized patients and fewer days of hospitalization. The personalized educational strategies, presented in a diagram, are described in two of their applications: week-long hospitalization and outpatient treatment, both combined with long-term educational follow-up, involving external physicians and nurses; an evaluation method is currently underway. The strategy underpinning the projects must incorporate the concepts of community-based facilities, the essential role of patient education, and the training of healthcare providers in their educational role.
