The Relevance of Regional Surveillance for Psittacosis: Pays de la Loire and Brittany. Evaluation of an Experimental Surveillance System and the Diagnostic Value of Laboratory Tests

A study on psittacosis conducted in 2008–2009 in western France identified 114 hospitalized cases suspected of psittacosis, 81% of which were from the Pays de la Loire and Brittany regions. Seven clusters of cases were detected in the Pays de la Loire region at duck farms, poultry operations, and during a bird fair. This report presents an assessment of the relevance of psittacosis surveillance, which was based on surveys of stakeholders’ perceptions of the system, an analysis of the value of investigations into clusters of cases, and a study of the diagnostic value of laboratory tests. Overall, clinicians’ perceptions were positive, with greater compliance and more reports among those practicing in Vendée, Maine-et-Loire, and Loire-Atlantique. Responding veterinarians agreed that the reporting process was simple. However, the evaluation highlighted a lack of completeness in the reports. The conclusions of epidemiological, serological, and veterinary investigations during epidemic episodes were limited due to the low number of cases, difficulties with biological diagnosis, and the culling of animals. These investigations provided an opportunity to raise awareness among human and animal health partners and to stimulate discussion regarding safety measures in poultry farms. Biological confirmations were rare, and discrepancies in results between PCR and serology were almost systematic. We propose focusing surveillance efforts on strengthening the reporting of occupational diseases, establishing a protocol for investigating clusters, and conducting studies on diagnostic methods.
