Validation of the French version of a screening tool for domestic violence against women, the WAST (Woman Abuse Screening Tool)

Introduction - Healthcare professionals underestimate the prevalence of domestic violence, and few of them report effectively screening for and managing victims. Furthermore, they lack a simple, validated tool in French. Our objective was therefore to validate the French version of the Woman Abuse Screening Tool (WAST) questionnaire. Materials and Methods - A case-control study was conducted in the Department of Forensic Medicine at the Clermont-Ferrand University Hospital and in two Women’s and Family Rights Information Centers. Female victims and non-victims completed the WAST questionnaire and a questionnaire regarding their comfort level in completing it during the study and during a hypothetical consultation with their primary care physician. An analysis of the psychometric properties and diagnostic performance of the WAST was performed. Results - The acceptability of the WAST was very good, with a response rate exceeding 95%. The diagnostic performance for a cutoff value of 5 was excellent: the area under the ROC curve was 0.99, sensitivity was 97.7%, and specificity was 97.1%. Comfort levels were significantly lower among victims than among non-victims. Both groups felt more comfortable completing the WAST during a study than during a consultation. Discussion - The French version of the WAST screening tool is valid and well-accepted for identifying domestic violence in routine practice. It can help healthcare professionals identify women experiencing violence early on and thus optimize their care.
