Retrospective data on radiological screening and follow-up for active tuberculosis among newly arrived foreign nationals, collected by the Isère Tuberculosis Control Center in 2018
Introduction - In 2018, the Isère Tuberculosis Control Center (CLAT 38) strengthened its strategy for the screening and management of active tuberculosis (ATB) among newly arrived foreign nationals. This article presents the results of radiological screening and follow-up for ATB in this population in 2018. Method - This retrospective study includes adults, families, and unaccompanied minors (UMs) who arrived in France less than two years ago and underwent a screening chest X-ray (CXR) or were diagnosed with TB in Isère in 2018. Results - In 2018, 1,694 adults and accompanied children and 1,830 UMCs underwent a screening PX. Clat 38 detected 10 TB cases among the population of adults and accompanied children out of a total of 22 TB cases in Isère within this population (Number Needed to Screen (NNS) of 169 RP for one TB case) and 15 among JMNA out of a total of 18 (i.e., an NNS of 143 and a prevalence of 837/100,000 in this population). Treatment interruptions were observed in 9 JMNA, 4 of whom were lost to follow-up. The detected TB cases are predominantly bacteriologically negative. Discussion and conclusion - The study shows a high prevalence of TB, as well as the relevance of RP in screening this population. A better understanding of the progression of bacteriologically negative TB cases is necessary. Administrative and social difficulties related to the status of JMNA made adherence to anti-TB treatment difficult. Beyond isolated changes in CLAT practices, national and local institutional frameworks must contribute to improving the care of newly arrived foreign patients in precarious situations with active TB through better coordination and consideration of their social circumstances (administrative status, housing, food).
