A series of lactobezoar cases in premature infants hospitalized at the Martinique University Hospital from July to October 2013

Introduction: On October 3, 2013, a pediatrician from the pediatric and neonatal intensive care unit at the Martinique University Hospital reported to the Regional Health Agency (ARS) an abnormal and sudden increase in severe gastrointestinal incidents among premature infants: seven cases of lactobezoar were recorded in that unit over a three-week period, one of which resulted in death. Methods: The Antilles-Guyana Health Authority conducted an exploratory investigation with the following objectives: - to describe the cluster by identifying common exposures and focusing on exogenous and endogenous risk factors reported in the literature; - to guide the development of an etiological hypothesis and further epidemiological investigations. The case definition for lactobezoar was an infant hospitalized in this department between July 1 and October 3, 2013, with: - gastric perforation, necrotizing enterocolitis, or enteropathy; - an abnormal radiological image showing a mass suggestive of a milk conglomerate. Results: A total of 7 cases of lactobezoar were confirmed between July 1 and October 3, 2013, one of whom died. All were preterm, male, with an average weight of 1780 g. Half of them weighed less than 1940 g. On average, they measured 43 cm at birth. Discussion: The exploratory study identified 7 cases of lactobezoar during the study period, occurring within a 37-day span. This absolute incidence appears particularly high compared to data in the literature. This investigation did not identify any hypothesis regarding the origin of the phenomenon. (R.A.)
