In July 2016, blood donation was opened up to men who have sex with men: no impact on transfusion-related risk for HIV, HBV, and HCV
Since July 10, 2016, men who have sex with men (MSM) have been permitted to donate whole blood if they have not had sex with another man in the past 12 months. This article presents a review of epidemiological surveillance of HIV, HBV, HCV, and syphilis among blood donors to assess the impact of this change on epidemiological indicators. The rate of HIV-positive donations decreased from 0.10 per 10,000 donations during the period from January 1, 2014, to July 9, 2016, to 0.08 per 10,000 during the period from July 10, 2016, to December 31, 2018 (p=0.2); the proportion of MSM among HIV-positive donors increased from 45% to 55% (p=0.5), and the incidence of HIV decreased from 1.05 to 0.85 per 100,000 (p=0.5). Between 2014 and 2018, the rates of positive donations decreased for HBV and HCV and remained stable for HIV and syphilis. Over the 2016–2018 period, the residual risk was estimated at 1 potentially infected donation every two years for HIV, 1 donation per year for HBV, and 1 donation every 11 years for HCV. Between the 2013–2015 and 2016–2018 periods, the residual risks for these three viruses remained stable. Based on the stability of these indicators before and after July 10, 2016, the Minister of Health called for a broader opening of blood donation to MSM. In 2019, HIV transmission risk analyses were conducted for two scenarios: a four-month deferral for MSM or a deferral for MSM who had had more than one sexual partner in the four months prior to donation. The first scenario was adopted in July 2019 and implemented on April 2, 2020.
