Epidemiological evaluation of organized colorectal cancer screening in France. Review of the pilot program: updated data as of November 1, 2007

In France, colorectal cancer is a major public health issue, both in terms of the number of new cases and the number of deaths each year. Launched in 23 pilot departments, the organized colorectal cancer screening program will be expanded nationwide in 2009. It is based on the Hemoccult® test (screening for occult blood in stool), offered every two years (invitation campaign) to men and women aged 50 to 74 who are at moderate risk for this cancer. A full colonoscopy must be performed systematically in the event of a positive test result. The departmental screening management organizations are responsible for inviting the target population and ensuring the smooth follow-up process. The first letter invites the population to visit their primary care physician (consultation not covered by the organized screening program), and it is the primary care physician who decides, based on the individual’s medical history, whether or not to perform a Hemoccult® test. A first follow-up letter is sent three months after the initial invitations if the test has not been performed. If necessary, this is followed six months later by a second reminder letter containing the test. The Directorate General of Health and the National Cancer Institute jointly oversee the program at the national level. A set of specifications published in 2006 describes its organization. The Institute for Public Health Surveillance is responsible for its epidemiological evaluation and measures various indicators based on data regularly collected from administrative units. An initial evaluation of the pilot program was conducted in 2006, and an update was published in 2007.
