Impact of the Implementation of the Organized Cervical Cancer Screening Program on Coverage Rates in France: A Comparative Analysis of the 2015–2017 and 2020–2022 Periods

Introduction – In 2018, a National Organized Cervical Cancer Screening Program (PNDOCCU) was launched in France to reduce the incidence and mortality of cervical cancer (CC), as well as disparities in access to screening. This article examines screening coverage rates, both for spontaneous and invitation-based screening, to assess progress toward national goals of reducing regional and age-based disparities. Method – Using data from the National Health Data System (SNDS) and estimates from the National Institute of Statistics and Economic Studies (INSEE), the study measured three-year screening coverage rates for CC among women aged 25 to 65. Results – The results reveal an increase in coverage rates between 2015–2017 and 2020–2022 in most regions, ranging from 0.1% to 23.7%. Regional variations ranged from 18.7% to 61.5% for 2015–2017 and from 16.0% to 66.3% for 2020–2022. During the 2020–2022 period, among women screened by invitation, 11.3% were aged 20–25, while 18.9% were aged 60–65. Disparities in coverage were observed, with 47% for those aged 60–65 and 67% for those aged 25–29. Conclusion – Despite the progress observed, significant age-based and geographic disparities persist, and challenges remain in expanding and standardizing access to screening across the country.
