PrEP for Migrants: Are We Really There Yet?

Introduction - Nearly 40,000 people are currently using pre-exposure prophylaxis (PrEP) in France, the majority of whom are men who have sex with men (MSM). Migrants—particularly women and men from Africa and the Caribbean—remain underrepresented in PrEP use. Method - Three cross-sectional surveys of prescribers conducted in 2018, 2019, and 2021 assessed the frequency of PrEP prescriptions for migrants (of all sexual orientations), the barriers encountered, and opportunities for improvement. The experiences of certain respondents who have developed innovative approaches are then described. Results - Between 60 and 113 professionals responded to the annual surveys. The frequency of prescribing to at least one migrant increased from 27% in 2018 to 70% in 2021. A large proportion of prescribers prescribed it to fewer than 5 migrants. Migrant MSM represent a significant portion of active patients. The proportion of non-MSM migrant women, men, and transgender individuals receiving PrEP has gradually increased over four years. The main obstacles encountered by prescribers include difficulty in monitoring migrants on PrEP, migrants’ lack of knowledge about PrEP, barriers to accessing their rights, and their lack of interest in this prevention tool. Conclusion - The prescription of PrEP for migrants is a reality in France, but remains rare and primarily benefits MSM born abroad. It must be encouraged and facilitated through various innovative approaches.
