Echinococcus spp. infections in mainland France: trends in the incidence of new hospitalizations from 2006 to 2022
Introduction – Alveolar echinococcosis (AE) is a zoonosis caused by human infection with the parasite Echinococcus multilocularis, while cystic echinococcosis (CE) is caused by Echinococcus granulosus. We used data from the Medical Information Systems Program (PMSI) to analyze the geographic and temporal trends in the incidence of new hospitalized cases of echinococcosis in mainland France. Methods – Patients with at least one diagnosis coded according to the International Classification of Diseases, 10th Revision (ICD-10), for Echinococcus infection between 2006 and 2022 were classified as having infections with Echinococcus multilocularis (EA), Echinococcus granulosus (EK), or Echinococcus without specification of species (Esp). The annual incidence was calculated as the number of new hospitalized cases with Echinococcus infection each year, relative to the population. Trends in incidence were analyzed using linear regression. Results – A total of 4,454 patients were included: 407 EA patients, 1,546 EK patients, 160 patients with EA and EK codes, and 2,341 Esp patients. EA patients were primarily identified in Bourgogne-Franche-Comté, Grand Est, and Auvergne-Rhône-Alpes. The average annual incidence in mainland France was 0.04 hospitalized cases per 100,000 inhabitants, and was decreasing. The majority of EK patients resided in Île-de-France, Auvergne-Rhône-Alpes, Provence-Alpes-Côte d'Azur, and Grand Est. The average annual incidence was 0.14 per 100,000 inhabitants, and was decreasing. Conclusion – National trends showed a decrease in the number of new hospitalizations for EA or EK between 2006 and 2022. No region showed an increase in the incidence of hospitalizations. Inaccuracies and possible errors in diagnostic coding limit the interpretation of the observed decreases.
